Position Details

• Role: Social Worker – Child in Need

• Contract: Full-Time Locum

• Start Date: ASAP

• Pay Rate: £37.52 per hour

• Hours: 37 hours per week

• Working Pattern: Monday to Friday

Key Responsibilities

• Manage a caseload of children and families within the Disabled Children's Team.

• Complete Child and Family Assessments and specialist disability assessments within statutory timescales.

• Assess, implement, and review short breaks packages based on assessed need.

• Work jointly with Health colleagues on Continuing Care assessments and joint-funded care packages.

• Undertake direct work with children and young people, ensuring their views and wishes remain central to all planning.

• Complete statutory visits, reviews, and outcome-focused care planning.

• Prepare reports for child protection conferences, court proceedings, reviews, and multi-agency meetings.

• Work collaboratively with families and partner agencies to safeguard children and achieve positive outcomes.

• Contribute to the team's duty service by responding to referrals and safeguarding concerns.

• Maintain accurate case records using Mosaic and support service improvement initiatives.
