About the Role

Health Now Complex Care is looking for experienced and compassionate Complex Care Support Workers to join our team in Grimsby, supporting an individual with complex physical healthcare needs within their own home.

This is a rewarding role where you will provide high-quality, person-centred care and support, helping the individual to remain safe, comfortable and as independent as possible.

Key Requirements

To be considered for this role, you must have previous hands-on experience with:

PEG care

Tracheostomy care

Cough Assist

Supporting individuals with complex physical healthcare needs

Following care plans, risk assessments and clinical guidance

Recognising and escalating changes in an individual's health or presentation

Working confidently as part of a wider care and clinical team

Desirable

The following would be advantageous but are not essential:

Full UK driving licence and access to a vehicle

Enhanced DBS already in place

Up-to-date mandatory and/or clinical training

Previous experience working within a complex care or community setting

Your Responsibilities

As a Complex Care Support Worker, you will be responsible for:

Providing safe, compassionate and person-centred care

Supporting with PEG and tracheostomy care

Using Cough Assist equipment safely and appropriately

Supporting with personal care and daily living activities

Monitoring the individual's health and wellbeing

Administering medication where trained and competent to do so

Maintaining accurate and professional care records

Following all care plans, policies and clinical procedures

Working alongside nurses, carers and other healthcare professionals

Escalating any concerns or changes in condition appropriately
