SW Locums is recruiting an experienced Adult Social Worker to join the Hospital Discharge Team at the London Borough of Hackney. This is a fantastic opportunity for a skilled practitioner to support adults through a crucial stage of their care journey and help achieve positive outcomes in the community.

Role Details

• Location: London Borough of Hackney

• Start Date: ASAP

• Contract: Full-time locum

• Working Pattern: Monday to Friday

• Pay Rate: £32.00 per hour (Umbrella)

About the Role

The Adult Social Care Hospital Social Worker plays a key role in supporting patients, particularly older adults and those with complex health and social care needs, to safely transition from hospital back into the community.

You will work closely with individuals, families, health professionals and partner agencies to assess needs, coordinate support and help prevent avoidable hospital admissions and readmissions.

Key Responsibilities

• Complete Care Act assessments and identify appropriate care and support needs.

• Support safe and effective hospital discharge planning.

• Work with adults, families and carers to develop personalised support plans.

• Collaborate with health colleagues, hospitals and community services.

• Assess risks and ensure appropriate safeguarding actions are taken where required.

• Promote independence, choice and wellbeing for adults returning home.

• Maintain accurate case records and complete statutory documentation.

What We’re Looking For

• Qualified Social Worker with current Social Work England registration.

• Strong Adult Social Care experience.

• Previous hospital discharge or community-based experience is highly desirable.

• Good understanding of the Care Act 2014, safeguarding and strengths-based practice.

• Ability to manage a caseload and work effectively within a multidisciplinary team.
